Please read ALL of the information below before deciding if you need to complete this form.

The Department for Education (DfE) is providing us with some additional laptops to help disadvantaged children and families access remote education during coronavirus (COVID-19). They are also helping disadvantaged children who need an internet connection to get internet access.

To apply for one of these devices your child needs to be classed as ‘disadvantaged’.

The Department for Education (DfE) defines ‘disadvantaged pupils’ as:

* Pupils eligible for Free School Meals OR who have been in the last six years;
* Looked after children (LAC), defined in the Children Act 1989 as one who is in the care of, or provided with accommodation by, an English local authority (LA).
* Children who have ceased to be looked after by an LA in England and Wales because of adoption, a special guardianship order, a child arrangements order or a residence order.

Please complete this questionnaire if you think that your child/children fall into one of these categories. Please complete for EACH CHILD in your family that attends St Oswald’s AND is in Year 3, 4, 5 or 6.
